To register for any of our new and exciting professional studies workshops, please print this registration form and clearly print your information, sign and fax your completed form to Lorna Wolfe at 604.599.2460. You may also mail your completed form to Kwantlen Polytechnic University, c/o Lorna Wolfe, 12666 72nd Avenue, Surrey, BC V3W 2M8, however it can take up to three weeks to be processed when sent via the mail. If you are registering for Consolidated Clinical, please call 604-599-2232 to make an appointment to register. Please note, all Professional Studies courses are non-refundable.
